The first hours matter most

For a fresh burn, immediate first aid changes the outcome more than anything done later. Cool the area under running water for twenty minutes. Not ice, which causes further injury, and not toothpaste, oil, ink or any household remedy, all of which are still applied regularly and all of which make assessment and treatment harder.

Remove clothing and jewellery from the area unless stuck to the skin, cover with a clean cloth or cling film, and seek assessment. Burns involving the face, hands, feet, joints or genitals, and any burn larger than the patient's palm, should be seen the same day.

Why scars tighten

Healing skin contracts. Across a flat surface that matters little, but across a joint it pulls, and over months the joint is drawn into a fixed position. That is a contracture, and it is the commonest reason burn patients lose function.

Pressure garments, silicone and splinting during the maturation phase reduce this considerably, which is why they are prescribed for many months and why compliance matters. Where a contracture has already formed, releasing it needs surgery, usually with a graft or flap to fill the gap that opens when the tissue is released.

Timing reconstruction

Most reconstruction waits until the scar has matured, which takes twelve to eighteen months. Operating on an actively remodelling scar tends to produce another poor scar.

There are exceptions where waiting causes harm. A contracture restricting a growing child's joint, one preventing the eye from closing, or one causing progressive deformity should be released early. Those decisions are made on examination rather than by a fixed timetable.
